Patient Forms
Save time in our office by completing your patient forms ahead of time. Simply download them, fill them out and bring them with you to your appointment.
Insurance
We are in-network with many insurance providers. We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits.
We will do everything we can to help you afford the treatment you need and want. For patients who require major work from our San Jacinto area dentist, we offer 3rd party financing with CareCreditOpens a new tab to CareCredit website.
Payment and Cancellation
Unless otherwise arranged, payment is due at the time of service. We accept all major debit and credit cards, cash, and checks.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice. We ask for this advance notice so that we can offer this appointment to another patient.
